What to Expect

A doctor on Dr. Attawar’s team will perform an inital review review of your medical history and past test results. A physical may also be conducted and additional tests may be ordered. After this, Dr. Attawar will meet with you and your family to discuss treatment options and begin the intake process.

Depending on your medical concern, your first visit may take several hours. Please plan accordingly.

What to Bring

We strongly recommend you bring all or as many of the following items as feasible for your first visit.

• Medical records

• Radiology CDs and reports - X-rays, CT scans, catheterization films, echocardiographs, stress test, and EKG results

• Lab test results

• A list of all medication you are currently on and their dosages

• Insurance card and any supplemental information

• Information about the doctor that referred you, if relevant

• Contact information of all the doctors currently involved in your care

• A list of questions or concerns you’d like to discuss

• Pen and paper or a tablet to take notes

• Completed cardiac pre-visit assessment form that will be emailed to you upon appointment confirmation
